Consent to use, collect, hold, process or disclose Personal Data/Sensitive Personal Data under Personal Data Protection Act 2010 (PDPA), Foreign Account Tax Compliance Act (FATCA) & Common Reporting Standard (CRS). 

  1. I/We consent the Company to use, collect, hold, process or disclose my/our Personal Data/Sensitive Personal Data* as defined in the Malaysian Personal Data Protection Act 2010 (PDPA 2010), pursuant to this policy contract, which shall for all intent and purpose to include but not limited to application form, counter-offer form, policy contract, endorsement etc. (“Policy Contract”), to its group of companies (within or outside Malaysia), respective employees, professional advisers or bodies including Life Insurance Association of Malaysia, Ombudsman for Financial Services, ISM Insurance Services Malaysia Berhad, intermediary or representatives, any third party, insurance or reinsurance companies, regulatory authorities whether or not relating to an intergovernmental agreement between the governments or regulatory authorities of two or more jurisdictions, external service providers who provide support or professional services to the Company including, but not limited to, website or email hosting services, information technology services, data security consultancy services, data analysis, legal services, accounting services, tax related services, administrative services and data back-up service (within or outside Malaysia) and/or to another country or jurisdictions in which I/we may be a tax resident, for purposes of (a) processing my/our application and verifying my/our eligibility for insurance and financial products and services, (b) provision and design of insurance, financial services or related products, (c) processing of any credit, medical, security and underwriting checks and insurance claims, (d) processing payment instructions (e) responding to my/our queries, (f) any other purpose in connection with the provision of any product or service to me/us, and/or (g.) any other purpose that may be deemed necessary or expedient by the Company in accordance with applicable laws and regulations, including but not limited to the purpose of complying with the requirement(s) of any governmental or regulatory authority, or court of law, of competent jurisdiction and in CRS, i.e., the Standard for Automatic Exchange of Financial Account Information in Tax Matters. I/We further confirm that I/we understand that both the Company and I/we am/are bound by the provisions of PDPA, FATCA and CRS and my/our consent to the Company disclosing my/our Personal Data/Sensitive Personal Data* to the regulatory authorities pursuant to FATCA and CRS. 
     
  2. a) I/We consent the Company to use, collect, hold, process or disclose my/our Personal Data/Sensitive Personal Data* to any hospital, clinic, physician, specialist, insurance or reinsurance companies, professional adviser or bodies, Life Insurance Association of Malaysia, Ombudsman for Financial Services, ISM Insurance Services Malaysia Berhad, organization, institution or person(s) and authorized intermediary or representatives for the purpose of processing this Policy Contract. 
    b) I/We consent to any hospital, clinic, physician, specialist, insurance or reinsurance companies, professional adviser or bodies, Life Insurance Association of Malaysia, Ombudsman for Financial Services, ISM Insurance Services Malaysia Berhad, organization, institution or person(s) and authorized intermediary or representatives to use, collect, hold, process or disclose my/our Personal Data/Sensitive Personal Data* to the Company and/or its authorised representatives for the purpose of processing this Policy Contract. 
     

*Note: 

  1. Personal Data/Sensitive Personal Data may include Proposer, Life to be Assured, Contingent Owner and/or Trusteeʼs name, gender, religion, race, identification number, address, phone number, email address as well as non-public information including details of condition or history of medical, health and hospitalization, financial, familial and non-familial information and any updated information of the same for the applicable product or service and such information as defined in “personal data” and “sensitive personal data” of the PDPA. 
  2. As stated above, the Company shall not disclose the Personal Data/Sensitive Personal Data without prior consent of the Proposer, Life to be Assured, Contingent Owner and/or Trustee. 
  3. The Proposer, Life to be Assured, Contingent Owner and/or Trustee has the right to access his/their Personal Data/Sensitive Personal Data and the Company has the right to impose fee for this purpose. The Proposer Life to be Assured, Contingent Owner and/or Trustee shall be allowed to make any update or correction to his/their Personal Data/Sensitive Personal Data through a written request to the Company. 
  4. The Company shall take reasonable steps to protect the Personal Data/Sensitive Personal Data from any unauthorized access or misuse and in ensuring accuracy of the Personal Data/Sensitive Personal Data at all times and shall not keep the Personal Data/Sensitive Personal Data longer than necessary for the purpose of this application and/or proposal and maintenance of the Policy Contract.
