Confirmation of Understanding

 

I am aware that the product provides coverage/pays out benefits on the following:

(a) Death Benefit

(b) Total & Permanent Damage Benefit 

(c) Accidental Death Benefit

I am aware that the product does not provide coverage/ pay out benefits on the following: 

(a) Death benefit is NOT payable if the Life Assured dies due to dueling, the hand of justice, or committing or attempting to commit an unlawful act under Malaysian Law.

(b) Additional Accidental Death Benefit is NOT payable if death is caused by assault or murder, riot, suicide, being under the influence of alcohol, drug abuse, pregnancy, pre-existing disabilities, dangerous pursuits and occupation inclusive but not limited to boxing, diving and acrobat.

(c) Total and Permanent Disability (TPD) Benefit is NOT payable if TPD is caused by attempted suicide, self-inflicted injury, being under the influence of drugs and liquor, engaging in hazardous activity, engaging in submarine voyage, performing duties in military, pre-existing illness or accident, dangerous pursuits and occupation inclusive but not limited to boxing, diving and acrobat.

The list is non-exhaustive. Please refer to the policy contract for the fill list of exclusions under this policy.

I am aware that:

(a) If I change my mind, I have 15 days to return the policy/certificate after the date it has been delivered to me and I can obtain a refund.

(b) I can make a nomination.
