Should provide, orally or in writing, basic information such as: the misconduct itself, the person(s) involved, when it occured, and who is/are affected by the misconduct.
Must have firsthand knowledge of facts disclosed in report.
Should not be discouraged from making a report because they are unsure as to whether there is sufficient evidence to support their allegations.
